Home Ventilator Therapy Program

- Comprehensive clinical monitoring: Benefit from both in-person and remote monitoring using platforms such as Care Orchestrator with Philips DirectView, VOCSN MultiView, and ResMed AirView.
- Robust reporting capabilities: Access a wide range of reporting options, including EMR integration, readmission analysis, compliance tracking, and financial impact reporting.
- Individualized care plans: Individualized care plans are created and maintained in collaboration with the patient's physician and respiratory care team.
- Advanced monitoring options: Utilize transcutaneous or end-tidal CO2 monitoring for accurate respiratory assessment.
- Dedicated respiratory support: Rely on our in-house respiratory therapist available during business hours for troubleshooting, as well as a 24/7 on-call hotline for any patient or caregiver questions and concerns.
- On-site training and education: Receive on-site comprehensive training and education for patients, caregivers, and referral sources.
- Diverse equipment options: Choose from a wide range of ventilators, nebulizers, high-frequency chest wall oscillation vests, and other respiratory equipment options to ensure patients receive the most suitable therapy for their individual needs.
- Risk assessments and tailored visits: Risk assessments are completed for each patient to determine the frequency of respiratory therapy visits, ensuring every patient is seen when they need to be to address clinical needs.
Med Advantage Pulmonary Pathway
Med Emporium utilizes the Med Advantage Pulmonary Pathway to provide advanced patient treatment through well-defined navigation, open communication and tracking of evidence-based success.
- MED: Identify complex respiratory patients and evaluate the likelihood of readmissions. Work alongside discharge planners, pulmonary specialists, and clinical patient advocates to receive proper documentation.
- ADVANTAGE: Stay in contact with case management and providers for authorization and coordination of care upon discharge. Initiate care (introduction of device and preliminary education) while inpatient or upon discharge. After setup, patients are monitored daily (if applicable), via phone contact and regularly scheduled clinical visits.
- PULMONARY PATHWAY: During the regularly scheduled home visits, respiratory therapists work with patients covering specific educational topics and continuing dialogue to ensure proper patient success.
